We provide a comprehensive consultation at our Cardiff Bay Clinic covering all aspects of prostate problems and prostate pain, such as prostate cancer, benign (non-cancerous) prostate enlargement and prostatitis. Prostate tests available include – DRE, PSA, PCA3 test (urine test), transrectal ultrasound (TRUS), flow studies, cystoscopy and urodynamics. Prostatic biopsy if required will be arranged promptly.

For benign prostate disease as well as conventional transurethral resection (TURP) we offer photoselective vapourisation of the prostate (Greenlight laser).

Haematuria assessment / Blood in Urine

Blood in the urine (haematuria) may indicate significant problems within the urinary tract. This ‘one stop’ visit attempts to exclude worrying causes relieving anxiety swiftly. A urology clinic appointment will include seeing your urological consultant along with an ultrasound scan, urine and blood tests and a cystoscopy (bladder inspection) under local anaesthetic. Other tests are occasionally required and can be organised as required.

Erectile Dysfunction

Erectile dysfunction (ED) is the inability to get and maintain an erection that is sufficient for satisfactory sexual intercourse. It can also be known as impotence. ED is a very common condition, particularly in older men.

It is estimated that half of all men between 40 and 70 years of age will experience ED at least once. ED can have a range of causes, both physical and psychological.

Urinary Incontinence and Treatments for Incontinence

Incontinence can be embarrassing and restrictive. Symptoms of stress, incontinence, overactive bladder (urinary urgency and frequency) can be fully assessed and investigations carried out in one visit to our private clinic in Cardiff when requested. Other investigations can be arranged including urinary flow studies, cystoscopy, urodynamics.

A full range of therapeutic approaches to incontinence is available from lifestyle modifications, medications and BOTOX injections along with periurethral bulking, ‘sling surgery’ – TVT, autologous slings (own tissue), TOT and coloposuspension.

Recurrent Urinary infection / Cystitis

Up to 50% of women may have experienced an episode of urinary infection by their twenties. Urinary infections are much less common in men. The symptoms are familiar with urgency, frequency and dysuria being prominent. Occasionally blood may be seen (haematuria) in the urine. It is important to rule out any underlying predisposition to urinary infection, however in the main we rarely find a sinister cause. Some of the investigations relating to urinary infection and cystitis that your consultant may recommend include the following: urine microscopy, urine cytology, urine flow test and scan, ultrasound scan, KUB x-ray, IVU, cystoscopy and urodynamic studies.

Kidney stones

Kidney stones may be anywhere in the urinary tract and treatment depends on the size, location and number of stones. Kidney stones may present as focal pain, blood in the urine and/ or infection.

Surgical therapy is not always necessary since some kidney stones will pass spontaneously, other specific types of stones may be dissolved using pharmacotherapy whilst others may be suited to fragmentation with external shock waves (lithotripsy). Larger, more complex kidney stones may require surgery which is most frequently undertaken with internal cameras (endoscopes) or keyhole surgery (PCNL).

Thorough evaluation of any patient who has urinary stones is necessary in the form of blood, urine and stone fragment testing. In a patient who has recurrent stones, and/or a strong family history, and/ or is less than 30 years of age a detailed metabolic evaluation with additional blood and urine tests is mandatory. Appropriate advice can be given to minimise subsequent kidney stone formation since stone recurrence rate approximates to 50% within 10 years.

Paediatric Urology - Circumcision and testicular problems in children

Circumcision

The majority of child referrals to our chilrens urology clinic include the management of the foreskin and assessment of the testicle(s).

Male circumcision is the most common surgical procedure in the world. It may be performed for clinical reasons or to comply with religious/cultural practice.

Cultural / religious circumcisions are not uniformly available on the NHS (where they are performed by medical practitioners) but are performed in the community by general practitioners and non-clinicians. At the Vale Healthcare private clinic we provide a consultant-delivered circumcission service to include assessment and treatment as required.

Undescended testicles

Undescended testes are common, affecting 1% of one year old males. Expert assessment is required to determine whether the testicle is present undescended or absent, retractile or ectopic. Undescended testicles, if present, should be mobilised and fixed in the scrotum before the age of 2 years to ensure its optimal development.

We provide a prompt comprehensive consultant opinion at our state of the art private clinic in Cardiff and can accommodate patients at a convenient date to undertake appropriate surgery in the management of both undescended testis and the foreskin.

Testicular / Scrotal problems - Lumps and cysts on the testicles / scrotum

Men commonly have concerns or anxieties in these areas especially when it comes to ‘lumps’ or swellings that have appeared on the testicle or when they experience testicular pain. In the vast majority of cases testicular lumps and pain in the testicles are not dangerous conditions but testicular cancer and tumours need excluding. Early detection of testicular cancer is important and usually leads to a full recovery.

Testicular and scrotal examination and diagnosis can usually be achieved in a single consultation with one of our urology consultants, occasionally blood tests are requested and for further reassurance an ultrasound of the testicles and scrotum scan can be arranged by our specialists.

Common conditions of the testicle that we see include hydrocoele, epidydymal cysts and epididymo-orchitis.

Testicular cancer is much less prevalent, and affects around 2,000 men (mostly young and middle-aged) in the UK each year. It accounts for one or two in 100 cancers in men and is the most treatable of cancers with 95% of men making a full recovery after treatment.

Early detection and treatment for testicular cancer is usually very successful. The type of treatment you have depends on the cancer and how far it has spread.

Frequent problems of the scrotum  include varicocoele, sebaceous cysts or pigmented skin areas.

Vasectomy & Vasectomy Reversal

Vasectomy

Full counselling is provided prior to any vasectomy. The vasectomy is performed under local anaesthesia in our private clinic. Information will be given regarding follow up semen analysis. We appreciate an individual’s circumstances may change and vasectomy reversal can also be organised.

Reversal of vasectomy

This is done when a man wants to father a child after having had a vasectomy. The blockage is excised and the tube (vas) is rejoined. It is one of the options that will be discussed with you.

Penile cancer

Cancer of the penis develops in the skin or tissue of the penis. Cancer of the penis is very rare, but it is still very important to be aware of the signs and symptoms. If penile cancer is detected early, the chances of a complete recovery are very good.

The exact cause of cancer of the penis is unknown, but men over the age of 40 are at greater risk of the disease. If penile cancer is caught early enough, circumcision may be effective. In further stages a part of the penis may need to be removed.

If penile cancer is small, treatment should not have any affect on your sex life or your ability to urinate. Radiotherapy or chemotherapy may also be used to kill cancer cells and shrink tumours.

Peyronie's disease

Peyronie’s disease is a condition caused by damage to the penile tissue creating a scarring under the skin. Depending upon the size and location of the scarring, it can cause the penis to bend upwards, downwards or become indented.

Peyronie’s disease is fairly common, affecting approximately three per cent of men. It can occur at any age, but most often affects men in their forties and fifties.

Symptoms of Peyronie’s disease include:

  • Pain during erections
  • Thickening of scar tissue under the skin
  • Curve of the erect penis.
